The syndrome presents as a muscle spasm near the anus in the levator ani muscle.  The muscle of the pelvic floor may be fragile and pressured.   This weakened state does not allow for the levator muscles to support the surrounding structure which can lead to decrease blood flow in that area.   Continued decrease in blood can lead to acidic conditions which can cause inflammation in the region.  This ultimately lends itself to the discomfort and lack of muscle coordination in the pelvic muscles.
